Extremely rapid heart rates such as this may result in hemodynamic instability or cardiogenic shock. In some cases, the combination of an accessory pathway and abnormal heart rhythms can trigger ventricular fibrillation , a leading cause of sudden cardiac death.
The bundle of Kent is an abnormal extra or accessory conduction pathway between the atria and ventricles that is present in a small percentage between 0.
The AV node is capable of slowing the rate of conduction of electrical impulses to the ventricles, whereas the bundle of Kent lacks this capability.
When an aberrant electrical connection is made via the bundle of Kent, tachydysrhythmias may therefore result. WPW is commonly diagnosed on the basis of the electrocardiogram in an asymptomatic individual.
In this case, it is manifested as a delta wave, which is a slurred upstroke in the QRS complex that is associated with a short PR interval. The short PR interval and slurring of the QRS complex are reflective of the impulse making it to the ventricles early via the accessory pathway without the usual delay experienced in the AV node.
If a person with WPW experiences episodes of atrial fibrillation, the ECG shows a rapid polymorphic wide-complex tachycardia without torsades de pointes.
This combination of atrial fibrillation and WPW is considered dangerous, and most antiarrhythmic drugs are contraindicated. When an individual is in normal sinus rhythm , the ECG characteristics of WPW are a short PR interval less than milliseconds in duration , widened QRS complex greater than milliseconds in duration with slurred upstroke of the QRS complex, and secondary repolarization changes reflected in ST segment - T wave changes.
In individuals with WPW, electrical activity that is initiated in the SA node travels through the accessory pathway, as well as through the AV node to activate the ventricles via both pathways.
Since the accessory pathway does not have the impulse slowing properties of the AV node, the electrical impulse first activates the ventricles via the accessory pathway, and immediately afterwards via the AV node.
In case of type A pre-excitation left atrioventricular connections , a positive R wave is seen in V1 "positive delta" on the precordial leads of the electrocardiogram, while in type B pre-excitation right atrioventricular connections , a predominantly negative delta wave is seen in lead V1 "negative delta".
This has been seen in individuals with Ebstein's anomaly. Wolff—Parkinson—White syndrome is sometimes associated with Leber's hereditary optic neuropathy , a form of mitochondrial disease.
WPW carries a small risk of sudden death, presumably due to rapidly conducted atrial fibrillation causing ventricular fibrillation.
While the overall risk is approximately 2. The faster it can conduct, the higher the likelihood the accessory pathway can conduct fast enough to trigger a lethal tachycardia. Risk stratification is best performed via programmed electrical stimulation PES in the cardiac electrophysiology laboratory.
High-risk features that may be present during PES include an effective refractory period of the accessory pathway less than ms, multiple pathways, septal location of pathway, and inducibility of supraventricular tachycardia AVRT , atrial fibrillation.
Individuals with any of these high-risk features are generally considered at increased risk for SCD or symptomatic tachycardia, and should be treated accordingly i.
It is unclear whether invasive risk stratification with PES is necessary in the asymptomatic individual. Other methods of risk stratification include observing the ventricular rate during spontaneous atrial fibrillation on a lead ECG.
RR intervals of less than ms suggest a higher risk pathway. During exercise testing, abrupt loss of pre-excitation as heart rate increases also suggest a lower risk pathway.
People with WPW who are experiencing tachydysrhythmias may require synchronized electrical cardioversion if they are demonstrating severe signs or symptoms for example, low blood pressure or lethargy with altered mental status.
If they are relatively stable, medication may be used. People with atrial fibrillation and rapid ventricular response may be treated with amiodarone  or procainamide  to stabilize their heart rate.
Procainamide and cardioversion are accepted treatments for conversion of tachycardia found with WPW. AV node blockers should be avoided in atrial fibrillation and atrial flutter with WPW or history of it; this includes adenosine , diltiazem , verapamil , other calcium channel blockers , and beta blockers.
The definitive treatment of WPW is the destruction of the abnormal electrical pathway by radiofrequency catheter ablation.
This procedure is performed by cardiac electrophysiologists. Radiofrequency catheter ablation is not performed in all individuals with WPW because inherent risks are involved in the procedure.
When performed by an experienced electrophysiologist, radiofrequency ablation has a high success rate.

That was the case with this one. I really enjoyed the story line. The main character was a little annoying at times, but I got that she was quirky and supposed to be like that.
It was steamy and I am definitely interested to reading more from these authors. I don't think I could not enjoy in some way anything that includes wolf-lovin!!!!!!!!!!!!

Back to the story. Carlee is an FBI agent working on a case that may threaten her life. When she finally fell in love with someone, he broke her heart.
Surprisingly, her ex-boyfriend, Ryden, shows up to save the day. I loved the combination of a kick-ass FBI agent and an alpha male werewolf.
The plot was intriguing too, and there were plenty of hot romantic scenes to keep the reader entertained. Highly recommended to fans of erotic paranormal romance.

Knowing she was conceived and born for the soul purpose of saving her brother's life and rejected by her parents when her bone morrow was rejected as a suitable match.
Ryden Coulter grew up with a childhood of neglect and abuse. Now as an adult he is an Elite Delta soldier. The two fell in love and completed one another.
Then Ryden broke Carlee's heart by cutting her out of his life completely without giving her any reason.
Circumstances have brought them together again. Their lives and those of Ryden's team depend on what they do.
I am in love I am just not sure with which one yet. Cam Cassidy and Ruelle Channing have created such fantastic and realistic characters that I think it is easy to fall in love with them.
Ryden and Carlee are both alpha personalities with an ingrained need to to take care of those they consider themselves responsible for.
Yet they seem to be unable to see what is right in front of them where there personal lives are consider. Which with there backgrounds it is not surprising.
This is the book you hate to start when going to bed thinking you will only read the first chapter to unwind a bit so your tired body will sleep.
The reason for that is A mistake I made at one in the morning. At three in the morning I was yawning so much and had my eyes propped open with toothpicks because my mind would not let me wait till later to find out what happened to Carlee, Ryden and all the other characters.
If this is authors Cam Cassidy and Ruelle Channing first attempt at a paranormal romance, then all I can say is I can not wait to see what they do for an encore.
Now, when will that be do you suppose? Considering this is a first novel one would expect it to have moments where it lags or a scene is forced, but this one does not on a scale of one to ten it deserves at lease an eleven.

The double pleasure with this novel is you might say it is also a second chance at love being they have been together before but one of them threw it away to protect the one he loved.
Carlee has become puzzle solver immersing herself into the FBI crime lab becoming a forensic scientist after having her heart slammed shut by Ryden leaving her behind when he left for the service than dumping her.
Right off when I see I really do enjoy a good paranormal that comes filled with romance and adventure. Nothing better to get the blood flowing then mystery and danger in the mix with paranormal on top of that.
Inject it with romance and I have found my sweet spot to sit to journey somewhere with these two characters to see if mysteries will be solved that involve murder with it.
I learn right away what happened also with Ryden and why he walked away from Carlee even though she is always center most in his mind.
He is way changed not the same man she knew for sure but just like a man to think the one who loves you won't understand. Protecting her also at all costs cause he realizes also she is his mate becomes no one in his sight with this bringing the two of them together with help from trusted mates of his to rid her life of the danger that has entered.
This also causes them to sit their sights on him also because of what he is. Lots of fascinating secrets revealed with the answers at times under some noses.
Love to read about the heat between a wolf and his mate with the intensity that a good writer can give it comes forth in this one as these two rebuild a relationship which their love for each other becomes really strong not neither can deny no longer.
Gripping adventure on the edge of your seat reading contained forwith in this novel to go along with the heat this packs. This author brings it all together with a most satisfactory ending that leaves you glad you indulged and entered once again into the paranormal world.
